We had a similar situation happen on James car. We used a non OEM hex bolt when installing a clutch as we were in a jam. We used a lock washer, torqued it to spec and everything. It decided to make it's way out and rattle around inside the bell housing, that is until the ring gear smashed it in to the side of the tranny. Ours broke through and ruined the plastic speedo gear on the final drive though, that sucked!
