Wooa, this is pretty much my same story!!!!
My 98 B14 came with the non LSD tranny so I bough a 94 LSD model from one of the member here. It worked like a charm the first week but then, suddenly, got stuck on 3rd gear and byitself, it pop out and started to worked randomly bad during 3 days until I decided to take it out, right before taking it out, it would not want to get into 5th any more.
We opened it and shift forks inside unaligned, it got damage and the 5th gear also was damaged.
We decided to transplant everything from the B14 tranny to the B13 tranny except for the 2nd gear as it grinded a little bit. We put back all together and the car worked just fine, for only few days and now it's stuck on first gear.
We are going to take it down again today. Someone said it was wear the cause of the problem but I am not really into that.
What else do you think might be happening?
As soon as we take it apart and open it, I will post pics.
