Rust - check A pillars, fenders and rear strut towers (under the carpet), these are the most common area's for rust to start.

5th gear popout is easily enough to happen on a NX as it is on a sentra. load in 5th gear is a good test to see if it will pop out or not. Find a small hill that you can drive in 5th gear.

top of the strut towers or bottem? and im pretty sure since it has a JDM motor and tranny in it with "lower miles" it shouldnt have 5th gear pop out now that i think about it but i know a great hill to test it on

Check the T-tops for leaks by spaying it a car wash lightly. If you really power it right on the top, it WILL leak. It only needs to hold out rain. Unless of course you're buying a rare hard top model.

Ask if the waterpump has ever been replaced. They are a real b**ch to get at.

Take off the oil cap,and look in with a flashlight. You will be surprised how much you can see in there.
It isn't as good as taking off the valve cover, but if it is neglected,it will be obvious.
Other wise...popout and rust are the biggest concerns on an NX from my experience.
